For you as a carer having someone in Hospital can be a worrying time
Illness or disability can have a huge impact on family life, sometimes financially and certainly emotionally. Therefore, it is so important that the right support, advice and information is made available to you.
For some of you it may be the first time you have had to care for someone leaving hospital and all that it entails, therefore being well informed, knowing what services can help or how to go about things can increase your capacity to act on life’s issues inside or outside the health arena.
Swansea Carers Hospital Outreach Project has been developed to ensure that you as a carer, relatives, patients and hospital staff are able to access support and advice within a hospital setting, and on leaving hospital are equipped with the knowledge and confidence to self- advocate and access the services you need or know where you can get the support to do so.
The project covers Morriston, Singleton, Gorseinon and Cefn Coed Hospitals.
Manned information stands are to be found in the outpatients department of Morriston and Singleton, Ysbryd y Coed in Cefn Coed and the day room in Gorseinon Hospital. Information is available on the following days:-
- Morriston Hospital – Tuesdays & Thursdays
- Singleton Hospital – Wednesdays
- Cefn Coed Hospital – 1st & 3rd Thursday of the month
- Gorseinon Hospital – 2nd & 4th Thursday of the month
As a carer you may have to attend meetings about your loved ones future needs, make important decisions as to whether you CAN care for someone at home or have to consider alternative accommodation e.g. sheltered accommodation or a Care Home.
We have the information around these issues at our fingertips and can guide you through the process alongside those involved in your loved one’s care. In some cases, we can attend meetings with you or signpost you to appropriate support services. Sometimes referral to our in house services may be just what you need but we can also refer or signpost you to specialist services and organisations.
We also know that you may be worried about what would happen to the person you care for if you were taken ill or hospitalised, so we have put together an ‘Emergency Plan’ available on the link below or by contacting our office for a copy.
Don’t worry if you live outside the City & County of Swansea we have links with all other Carer organisations and will endeavour to signpost you for the appropriate help and support.
